A nurturing public support network for people, over 45, who, are primary carers for loved ones facing health challenges and end of life, or who are personally facing the same challenges.

Swan Song Community was inspired Kate's life experiences, the few years caring for her Mum and her work as a Doula. Along these journeys, Kate's learned the profound impact of discussing, planning, and documenting one's wishes to ensure holistic care during life's transitions.
